Academic Committee Petition Student requests and appeals, especially those outside of policy 33
Add/Drop/Withdraw Voucher For adding, dropping, and withdrawing from courses after the published deadlines (Note: Most courses can be added/dropped online through Student Web Services) 30
Additional Degree Discount For students who have already obtained a four-year degree and are taking more classes 211
Change of Program For students who wish to change majors or add/drop a minor or certificate  
Directed Study Application Individual course work completed under the direct supervision of a CUC professor 36
Letter Request Confirmation of enrolment for loans, grants, immigration, etc., fees may apply  
Music Directed Study Application For students who wish to receive credit for music lessons taken with a private instructor 209
Off Campus Study Application To be completed BEFORE taking a course at another institution for transfer into a CUC program of study 36
Prior Learning Assessment and Recognition To receive credit for knowledge gained outside of traditional learning settings, fees apply 31-32
Registration Withdrawal Form Complete withdrawal from all courses 30
Reschedule an Exam Request to reschedule a final exam due to illness, family emergency, or other compelling reasons, fees may apply 31
Satisfactory/Unsatisfactory Registration Used for core, cognate, or elective courses; credits apply toward a degree without affecting GPA 28
Spousal Audit Discount Spouses of full-time students may audit one course per term free of charge 211
Time-Conflict Waiver To be completed when two class times overlap (i.e. labs), alternate time arrangements must be made with one professor  
Transcript Request Official transcripts for CUC courses, fees may apply 38
